Job Summary:

The successful candidate will be responsible for providing skilled physical therapy services to patients in their homes, as ordered by the attending physician. This includes developing and implementing individualized plans of care, educating patients and their families, and ensuring the safety of patients.

Responsibilities:
  • Plan and implement individualized plans of care for patients in their homes
  • Provide skilled physical therapy services, including exercises, massage, heat, and water therapy
  • Educate patients and their families on disease management and prevention
  • Supervise and train physical therapy assistants and nurse aides as needed
  • Communicate effectively with patients, families, physicians, and other healthcare professionals
  • Maintain accurate and timely documentation of patient progress and care
Requirements:
  • Current license to practice as a Physical Therapist in the state of employment
  • Valid driver's license and auto insurance
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
