Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality nursing care to residents in our skilled nursing facilities. As a key member of our healthcare team, you will be responsible for coordinating and implementing nursing care plans, supervising and guiding clinical staff, and ensuring the overall well-being of our residents.

Responsibilities
  • Coordinate and Implement Nursing Care Plans: Develop and implement individualized care plans that meet the unique needs of each resident.
  • Supervise and Guide Clinical Staff: Provide guidance and oversight to licensed vocational nurses (LVNs) and certified nursing assistants (CNAs) to ensure they are providing high-quality care.
  • Administer Medications and Treatments: Accurately administer medications and treatments as prescribed by physicians, and document all interactions and reactions.
  • Conduct Resident Assessments: Conduct regular assessments of residents to identify changes in their condition and develop plans to address any needs or concerns.
  • Collaborate with Interdisciplinary Teams: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive care and effective communication.
  • Maintain Accurate Records: Maintain accurate and up-to-date records of resident care, including progress notes, medication administration records, and other relevant documentation.
Requirements
  • Education: Graduate of an accredited LPN/LVN school of nursing.
  • Licenses/Certifications: Valid LPN/LVN license in the state employed, and valid CPR certification.
  • Experience: One year of experience in nursing, preferably in a skilled nursing or long-term care setting.
