We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Giving Home Health Care Careers in Ojo Sarco, NM.

Job Summary:

As a Home Health Care Nurse, you will be responsible for administering patient care in accordance with a physician-established care plan. This role involves working one-on-one with patients in their residences, providing personalized care and support.

Key Responsibilities:

  • Administer patient care in accordance with a physician-established care plan
  • Work collaboratively with CNA's, HHA's, Case Managers, and patient physicians
  • Provide personalized care and support to patients in their residences
  • Advocate for patients and their families

Requirements:

• RN or LPN licensure in the state of New Mexico

• 1 patient, 3 hours weekly for 1 visit

• Offers contingent upon passing a background check
