As a dedicated Home Health Registered Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive skilled nursing visits, including admission, case management, and follow-up care, to ensure patients receive the best possible outcomes.
  • Collaborate with physicians to develop and implement individualized care plans, addressing the unique needs and goals of each patient.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to optimize patient care.
  • Coordinate with interdisciplinary teams, including physical therapists, occupational therapists, and home health aides, to ensure seamless care delivery.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care coordination.
  • Document patient progress, outcomes, and care plans accurately and thoroughly, adhering to regulatory requirements and industry standards.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational, communication, and interpersonal skills
  • Valid driver's license, auto insurance, and reliable transportation
