The Regional In-Home Care Coordinator is a key member of the Ascentria Care Alliance team, responsible for the day-to-day management of the Ascentria In-Home Care program in their territory. This role involves processing referrals for consumer-directed personal care services serving the elderly and chronically ill population.
Key Responsibilities:
- Manage and coordinate the delivery of in-home care services to clients in their territory
- Process referrals and conduct assessments to determine client eligibility for services
- Develop and implement care plans to meet the unique needs of each client
- Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination
- Monitor and evaluate the quality of care provided and make recommendations for improvement
Requirements:
- Strong organizational and communication skills
