Job Summary:
Cullman Regional Medical Center is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in a Full Time Home Care position. As an LPN, you will play a vital role in providing high-quality patient care and promoting health and independence in the comfort of patients' homes.
Key Responsibilities:
- Collaborate with physicians, patients, and their families, nurses, therapists, social workers, and other members of the patient care team to implement and follow a patient-specific care and treatment plan.
- Cooperatively utilize varied solutions to educate and promote the health and independence of patients.
- Assess, monitor, document, and report progress of patients' health and condition using required documents such as clinical progress notes using a clinician-preferred technology and within established timelines.
Requirements:
- Current, valid, unrestricted AL LPN license.
- Minimum of one (1) year of clinical experience as a Licensed Practical Nurse.
- Current CPR certification.
Preferred Qualifications:
- Acute care experience as a licensed nurse with the adult population in a medical-surgical environment.
