Job Title: Clinical Pharmacist Home Infusion

CarepathRx is a leading provider of home infusion services, and we are seeking a highly skilled Clinical Pharmacist to join our team.

Job Summary:

The Clinical Pharmacist will be responsible for providing pharmaceutical care to patients receiving home infusion therapy. This includes safely and correctly compounding sterile medications, initiating infusion therapies, and evaluating patient care.

Key Responsibilities:

  • Compound sterile medications and infusions for home, outpatient clinics, skilled nursing facilities, and physician/clinic environments.
  • Initiate infusion therapies and evaluate patient care, including documentation and fiscal and legal requirements.
  • Understand and comply with Quality Assurance, Performance Improvement, and accreditation guidelines to ensure excellence in patient care.
  • Assist with the referral process, including provision of medical information, patient education, and coordination of deliveries.
  • Participate in education and training of licensed healthcare professionals.
  • Act as pharmacist in charge while on duty.
  • Maintain working knowledge of equipment and supplies necessary for home and outpatient infusion.
  • Responsible for knowing and adhering to all home infusion center and company policies and procedures.
  • Participate in inventory control, ordering, warehouse organization, packing/shipping, equipment maintenance, and hazardous waste control.
  • Responsible for emergency on-call duties as required by center scheduling.

Requirements:

  • Proficiency in Microsoft Suite and pharmacy software program.
  • Active Pharmacist license in good standing with the Board of Pharmacy.
  • Experience in intravenous therapy.
  • Strong clinical knowledge.
  • Good verbal and written communication skills.
  • Ability to work in a team environment and cooperate with all departments within the home infusion center.

Preferred Qualifications:

  • BS in Pharmacy or PharmD.
  • Current and licensed pharmacist with the Florida Board of Pharmacy.
  • Must be in good standing with all State Boards of Pharmacy where licensure is presently or previously has been active.
  • Meet State Board of Pharmacy and accreditation organization requirements by obtaining continuing education needed to maintain license.
  • Ideal experience in intravenous therapy in a hospital or home infusion setting, including parenteral admixtures and patient clinical management.
  • Teaching experience with nurses and clients.
