Job Summary:This is a full-time LPN position working night shifts, with a Monday-Friday rotation and every other weekend rotation. The successful candidate will have a strong background in rehabilitation nursing and excellent communication skills.
Responsibilities:- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments and document patient progress.
- Participate in quality improvement initiatives and contribute to a positive work environment.
- BLS certification and active LPN state license.
- At least 2 years of LPN experience in a rehabilitation setting.
- Excellent communication and interpersonal skills.
