Job Summary

We are seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with Adara Home Health, you will have the opportunity to work with a diverse range of clients, providing personalized physical therapy services to help them achieve their goals and improve their overall health and well-being.

Key Responsibilities
  • Administer physical therapy evaluations and participate in the development of individualized treatment plans.
  • Provide physical therapy treatments, procedures, and diagnostic tests to clients under the direction of their physician.
  • Translate exercises into functional activities or activities of daily living, and provide instructions on the use and care of assistive devices.
  • Develop plans for modifying equipment, appliances, and the physical surroundings in clients' homes to promote safe and effective care.
  • Prepare clinical and progress notes, and report to physicians and case managers on clients' responses to treatment or changes in condition.
  • Maintain accurate records, including frequency of visits and client responses.
  • Instruct and supervise physical therapy assistants and home health aides as needed.
  • Provide in-service education programs for company staff, and participate in case conferences as indicated.
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination.
Requirements
  • Graduate of an accredited Physical Therapy program, currently licensed as a Physical Therapist in the state of Minnesota.
  • At least two years of experience in a healthcare setting, with home care experience preferred.
  • Strong interpersonal, communication, and teaching skills.
  • Excellent organizational and time management skills, with the ability to work independently.
  • Access to a dependable vehicle or public transportation for travel to multiple company business stops.
  • Valid driver's license and proof of car insurance when using a personal vehicle for company business.
  • Proof of negative mantoux or documentation of negative chest x-ray.
  • Current CPR certification is highly recommended.
  • U.S. Citizenship or evidence of valid Alien Work Permit.
  • Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services.
Physical and Environmental Demands

This position requires heavy to very heavy physical exertion, involving lifting clients, bending, stooping, stretching, and setting up equipment. Must be adaptable to a variety of environments and community settings, and able to drive or use public transportation in all types of weather.
