Job Title: Occupational Therapist - Skilled Nursing Facility

Aequor Technologies is seeking an experienced Occupational Therapist to join our team in a Skilled Nursing Facility setting. As an Occupational Therapist, you will be responsible for assessing patients' physical and mental abilities to determine rehabilitation needs, developing and implementing individualized treatment plans, and providing therapeutic interventions to enhance fine motor skills, coordination, strength, and mobility.

Key Responsibilities:

  • Assess patients' physical and mental abilities to determine rehabilitation needs
  • Develop and implement individualized treatment plans
  • Provide therapeutic interventions to enhance fine motor skills, coordination, strength, and mobility
  • Collaborate with a multidisciplinary healthcare team to ensure comprehensive patient care
  • Monitor and document patient progress, adjusting treatment plans as necessary

Requirements:

  • Strong understanding of occupational therapy principles, especially in the context of a Skilled Nursing Facility environment
  • Excellent communication and interpersonal skills for interacting with patients, families, and a multidisciplinary team
  • Proficiency in creating and implementing individualized treatment plans based on patient needs
  • Active [STATE] Occupational Therapist (OT) License in good standing
  • A minimum of 2 years of experience as an Occupational Therapist or in a similar role
