Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Care team at UF Health. As a member of our treatment team, you will provide direct patient care to assigned patients under the direction and supervision of a Registered Nurse and attending clinician.

Key Responsibilities
  • Administer medications and provide wound care, injections, and other treatment interventions as ordered by the attending clinician.
  • Monitor patient vital signs and report observations to the Registered Nurse, recognizing any deterioration of patient condition and collaborating with the RN to respond accordingly.
  • Perform phlebotomy, conduct bedside glucose monitoring, and implement preventative care as ordered in the individualized patient's treatment plan.
  • Provide patient and family health education based on assessed needs for indicated health conditions, medications, overall wellness, and directions for follow-up care.
Requirements
  • Technical diploma in nursing
  • 30hr IV Certification
  • 1 year LPN experience, with preferred experience in Home Health
