Job Summary:
We are seeking a compassionate and skilled Hospice Aide to join our team at St. Croix Hospice. As a valued member of our interdisciplinary care team, you will work under RN supervision to provide high-quality patient care and support.
Key Responsibilities:
- Provide patient care, including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene, while maintaining a sanitary and orderly environment.
- Offer positive, respectful, and confidential patient/family communication.
- Take and record vital signs, and assist with self-administered medications.
- Plan and prepare nutritious meals, including shopping and feeding the patient as needed.
- Assess and report patient's condition and changes to the RN Case Manager.
- Assist in ambulation, exercise, transfers, positioning, and support a complete range of motion and other therapy activities.
Requirements:
- At least 18 years of age.
- Active Certified Nursing Assistant (CNA) designation current with state-specific requirements.
- Licensed and insured driver with automobile in good working order.
