Job Overview

UPMC Home Healthcare is seeking a dedicated Physical Therapist to join our team, focusing on providing exceptional care in the home environment. This role involves working primarily during daylight hours, Monday through Friday, with occasional weekend shifts.

Key Responsibilities:
  • Participate in compliance training and uphold the organization's ethical standards and policies.
  • Prioritize customer satisfaction and strive to deliver high-quality service in all interactions.
  • Engage in process improvement initiatives that enhance the quality of care provided by the home health agency.
  • Perform job duties in alignment with UPMC Home Healthcare's service management philosophy, ensuring respectful interactions with patients, families, and colleagues.
  • Commit to ongoing professional development through continuing education opportunities.
  • Contribute to on-call coverage as required by the agency.
  • Supervise and guide Physical Therapy Assistants in accordance with state regulations.
  • Collaborate with families, interdisciplinary team members, and physicians to ensure comprehensive patient care.
  • Ensure timely data synchronization and documentation of patient visits.
  • Complete all necessary documentation accurately, including assessments and care plans, in compliance with agency and regulatory standards.
  • Develop and implement individualized care plans in partnership with physicians and patients, adhering to medical orders and patient needs.
  • Conduct thorough evaluations and design effective treatment programs based on diagnostic assessments and patient capabilities.
Qualifications:

Applicants must have graduated from an accredited physical therapy program recognized by relevant professional bodies. Candidates with prior experience in physical therapy are encouraged to apply. A minimum of one year of clinical experience is preferred for independent practice.

Licensure and Certifications:
  • Valid Automotive Insurance
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification
  • Current Driver's License
  • Licensed Physical Therapist (PT)
  • Act 33 Clearance
  • Act 34 Clearance
  • OAPSA Clearance
