Rehabilitation Director/Physical Therapist Assistant

We are seeking a highly skilled Rehabilitation Director/Physical Therapist Assistant to join our team at Gordon Health & Rehab. As a key member of our rehabilitation services team, you will be responsible for overseeing the daily operations of our facility's rehab department, providing clinical case management oversight, and attending facility meetings as needed.
Key Responsibilities- Oversee the daily operations of the facility's rehab department, ensuring efficient and effective clinical case management.
- Provide direct patient care and additional support duties as needed.
- Responsible for scheduling patient care according to the patient Plan Of Care established by the Therapists.
- Collaborate with the Division Rehab coordinator to schedule appropriate rehab staffing to meet the patients' clinical needs.
- Contribute to a positive work team by sharing information, using problem-solving methods, and accepting new ideas, criticism, or advice from others.
- Provide ongoing collaboration with the skilled nursing facility interdisciplinary clinical team to facilitate effective and efficient clinical case management.
- Assist with the Insurance denial process and medical records requests.
- Support activities as introduced by skilled nursing facility and Rehabilitation Services.
- Demonstrate superior customer service through courtesy, consistent follow-up, and positive communication.
- Utilize continuing education assistance to achieve targeted quality improvements.
- Maintain a state-of-the-art body of knowledge in areas of clinical expertise through seminars, professional journals, and peers.
- Effectively demonstrate safe working practices.
- Reports for scheduled work periods on time.
- Performs other related duties and responsibilities that may become necessary or as directed by the immediate supervisor.
- Able to perform clinical and operational duties in accordance with Rehabilitation Services' guidelines and operational processes.
- Able to demonstrate Rehabilitation Services' inter-disciplinary approach to patient care to facilitate effective and efficient clinical case management utilizing evidenced-based clinical practice.
- Able to complete clear and accurate documentation in accordance with Rehabilitation Services, regulatory, licensing, payor, and accrediting requirements.
- Assists in recruiting, hiring, training, and supervision of a team that shall reflect Rehabilitation Services' commitment to outstanding patient care, including but not limited to performance reviews, corrective action, etc.
- Able to aid in building/maintaining Rehabilitation Services' reputation by contributing as a professional member of staff while upholding the Organization's Core values.
- At minimum, possess an Associate's degree in Physical Therapy.
- Possess a current, unencumbered, license to practice as a Physical Therapist Assistant in the state of Georgia.
- Prefer two (2) years of experience in a skilled nursing facility rehabilitation environment, or equivalent.
