We are seeking a highly skilled Clinical Staff Pharmacist to join our team at Klamath Tribal Health and Family Services. As a key member of our pharmacy team, you will play a vital role in providing quality pharmacy services to our patients.
Key Responsibilities- Provide clinical pharmacy services, including medication therapy management and patient education.
- Collaborate with primary care physicians and staff to improve patient outcomes.
- Develop and maintain pharmacy-related programs and a drug information center.
- Maintain current knowledge of state and federal laws, rules, and regulations.
- Accurately compound and dispense prescribed medications, drugs, and other pharmaceuticals.
- Assure appropriateness of drug therapy in selected medications.
- Provide consultation to patients and healthcare practitioners on drug therapy and patient care.
- Manage therapy for patients with complex medication regimens.
- Assist with supervising ancillary staff and ensuring prescription delivery is timely and accurate.
- Participate in interdisciplinary committees and medication use evaluations.
- Current License as Registered Pharmacist (Oregon Licensure required within 90 days of hire).
- PGY-1 Residency training and/or 2 years of applicable experience.
- Ability to work a flexible schedule with possible evenings.
- Ability to acquire and maintain BLS certification within 180 days of hire.
- Ability to submit to annual TB skin testing and adhere to KTHFS staff immunization policy.
- Ability to submit to a background and character investigation.
- Positive working experience with Native Americans in a related field.
- Board Certification.
- Demonstrated working knowledge of pharmacy system operations.
- Residency training in patient-centered managed care settings.
