Position Overview

The Progress Center Inc is currently seeking Full-time Case Managers across various locations. As a Case Manager, you will play a crucial role in linking adults with intellectual and developmental disabilities to essential community resources that encourage autonomy and strengthen community ties. The primary goal of Case Management Services is to assess individual needs in medical, social, educational, and other areas, identify necessary services, and facilitate access to those services. This role encompasses intake and assessment, care plan development, coordination and advocacy, as well as ongoing monitoring and evaluation.

Position Details:

  • Full-Time, 40 hours per week
  • Monday to Friday with a flexible schedule

Key Responsibilities:

  1. Conduct assessments and/or coordinate the assessment of the client's medical, social, educational, and other needs.
  2. Develop and execute a personalized care plan in collaboration with the client, their guardian, and other team members.
  3. Coordinate with service providers and resources as outlined in the care plan.
  4. Connect clients with relevant agencies, community resources, and informal support networks.
  5. Monitor client progress towards achieving the goals set in the person-centered plan, including regular evaluations of the plan itself.
  6. Engage in team meetings.
  7. Perform additional duties as outlined in the policies and procedures manual and as directed by the supervisor.
  8. Maintain and update client information in electronic records.

Qualifications:

  1. Bachelor's degree in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing, or a related field.
  2. Proficiency in electronic software (Microsoft Office Suite) and electronic health records, along with strong writing, typing, and organizational skills.
  3. Strong interpersonal skills and a professional demeanor are essential.
  4. Adhere to all federal, state, and local regulations regarding services for individuals with developmental or intellectual disabilities or autism, including recipient rights and mandatory reporting.
  5. Comply with Office of Aging and Disability Services and Department of Human Services regulations.
  6. Demonstrate respect and add value to the lives of those we support.
  7. Complete all required training.

Education Requirements:

  • Bachelor's degree (Required)
  • Experience working with individuals with disabilities (Preferred)
  • Social Work License, LSW (Preferred but not required)
  • Valid Driver's License (Required)
