About the Role

We are seeking a highly skilled Social Worker, Care Coordinator, LSW/MSW to join our team at Hackensack Meridian Health. As a key member of our healthcare team, you will be responsible for coordinating, communicating, and facilitating the clinical progression of patient treatment and discharge plans.

Key Responsibilities
  • Assess patients admitted to the trauma service to identify potential discharge needs and develop individualized plans in collaboration with physicians and healthcare teams.
  • Facilitate communication and coordination between healthcare team members and involve patients and families in decision-making processes to minimize fragmentation of services and manage resources.
  • Provide social work interventions to patients and families to assist with coping with traumatic injuries and stress.
  • Maintain current information on community resources and refer patients to those resources that will enhance their lives.
  • Collaborate with multidisciplinary healthcare teams to support crisis intervention, counseling, and referrals, as well as abuse and neglect, guardianship, and psychosocial assessments.
Requirements
  • Master's of Social Work degree
  • Two years of experience as a social worker in a healthcare setting
  • Trauma Certification in Social Work required within one year of hire
  • NJ Licensed Social Worker
  • Licenses and Certifications Preferred: NJ Licensed Clinical Social Worker, Certified Case Manager or Accredited Case Manager Certification, Trauma Certification in Social Work
