We are seeking a highly skilled Medical Provider to join our team at Community Health Center, Inc. (CHC). As a Primary Care Provider, you will play a vital role in delivering high-quality medical services to underserved populations.

Key Responsibilities:

• Provide a full scope of medical services, including preventive care, diagnosis, and treatment of various health conditions.
• Collaborate with a multi-disciplinary team to offer a blend of individual and population-based care.
• Work in an innovative practice environment utilizing Advanced Access scheduling.
• Participate in practice-based research and quality improvement initiatives to enhance patient outcomes.

Requirements:

• MD/DO from an approved medical/osteopathy school and graduation from an accredited residency program in family practice.
• Board certification within 24 months of commencing employment at CHC and licensure in the State of Connecticut.
• Bi-lingual skills in English/Spanish preferred.
• We also accept NPs for this position.
