Job Summary

We are seeking a full-time Palliative Care Nurse Practitioner to join our team at AlignMed Partners. As a key member of our healthcare team, you will provide primary and urgent care services to nursing center residents in collaboration with other medical providers.

Responsibilities
  • Provide high-quality patient care to residents in our nursing centers
  • Collaborate with Center Leadership to achieve AlignMed Partners' mission of improving the lives we touch through the delivery of high-quality healthcare and everyday compassion
  • Work collaboratively with Medical Directors and Attending Physicians for patient-specific medical care
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • Graduate from an accredited School of Nursing program
  • Licensed as a registered nurse
  • Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP)
  • Minimum of two years experience working with adult or geriatric population
  • APRN with an unrestricted professional license and DEA, for each state in which they practice
