Job Summary:

The Nurse Practitioner will be responsible for providing comprehensive primary care services to patients, including conducting office visits, administering annual wellness visits, and coordinating care with other healthcare providers. You will work collaboratively with our care team to ensure that our patients receive high-quality, patient-centered care.

Responsibilities:
  • Provide exceptional primary care services to patients, including conducting office visits and administering annual wellness visits.
  • Coordinate care with other healthcare providers, including specialists and testing facilities.
  • Participate in population health leadership, focusing on preventive care and health promotion.
  • Assist the care team with phone triage and outreach.
  • Educate patients on their health conditions, care plans, and treatments.
  • Participate in Oak Street Health promotional activities.
  • Conduct home visits as needed.
Requirements:
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • US Work Authorization.
Preferred Qualifications:
  • Experience in primary care: internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
  • Passion for teamwork and collaboration.
  • Desire to be part of an innovative model focused on empirically-guided population health.
