Pinnacle Home Care Inc. is seeking a dedicated and compassionate Registered Nurse to join our team of skilled home health care providers. As a Registered Nurse, you will play a vital role in providing quality care to our patients in their homes.
Job SummaryThe Registered Nurse is responsible for planning, organizing, and directing home care services with an emphasis on community health education. The ideal candidate will have a strong commitment to providing quality care to our patients and their families.
Responsibilities- Conduct initial assessments of patients and families to determine home care needs, including physical assessments and histories of current and previous illnesses.
- Regularly re-evaluate patient nursing needs and make necessary revisions to the plan of care.
- Develop and implement care plans that incorporate therapeutic, preventative, and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by physicians and provide direct patient care as defined in the State Nurse Practice Act.
- Counsel patients and families on meeting nursing and related needs, and provide health care instructions as appropriate.
- Identify discharge planning needs and implement prior to discharge of patients.
- Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for assigned caseloads.
- Associate's or Bachelor's degree in Nursing with one to two years of recent acute care experience in an institutional setting. Two years of home health care experience preferred.
- Must be licensed in the State as a Registered Nurse.
- Current BLS certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Prolonged or considerable walking and standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
- This position requires that the employee is willing and able to meet the physical requirements as necessary.
- Visual acuity and hearing to perform required skills.
