Senior Resources of West Michigan is seeking a Registered Nurse Care Manager to join our team. The RN Care Manager will be responsible for conducting assessments of participants' functional and medical care needs, developing plans of care, and implementing services to assist participants in remaining in an independent home environment.
Key Responsibilities:
Conduct functional assessments of participant health needs
Develop plans of care in conjunction with participants, their allies, and physicians
Assist participants with setting service frequencies and durations, and implement approved plans of care
Monitor participants' conditions and adjust plans as needed
Maintain communication with participants' families and physicians
Requirements:
Bachelor of Science in Nursing or Associate Degree in Nursing with 2-3 years of experience
Current Michigan Registered Nurse license
CPR certification
Ability to work independently and in a team approach
