We are seeking a skilled Physical Therapist to join our interdisciplinary team at Center for Elders' Independence. As a Physical Therapist Per Diem Float, you will provide restorative and rehabilitative services to our elderly participants, promoting autonomy and quality of life.
Key Responsibilities- Collaborate with the primary care provider team and interdisciplinary team to evaluate participants and develop individualized plans of care.
- Provide physical therapy services, including exercise, soft tissue and joint mobilization, functional training, and gait training, to relieve pain and develop or restore function.
- Conduct initial assessments and reassessments of participants as required, and document thoroughly and accurately.
- Participate in interdisciplinary team conferences and contribute to the development of the plan of care.
- Provide in-service training to other members of the health team and maintain required certifications.
- Evaluate participants' home environments and make recommendations as needed.
- Participate in coordination for 24-hour care delivery to participants and delegate to and monitor the physical therapy care provided by Physical Therapy Assistants as appropriate.
- Possession of a minimum of a Bachelor's degree and graduation from a program of Physical Therapy approved by the Commission on Accreditation in Physical Therapy Education.
- Current licensure as a Physical Therapist by the Physical Therapy Board of California.
- Preferred, one (1) year of experience working as a Licensed Physical Therapist with the frail elderly population.
- Current CPR and First Aid certification on file.
- A valid California driver's license, current vehicle insurance, and access to a reliable vehicle.
