Job Title: Medical Case Manager

We are seeking a highly skilled Medical Case Manager to join our team at Positive Impact Health Centers INC. As a Medical Case Manager, you will play a critical role in providing medical case management services to eligible adult clinic patients, following applicable funding guidelines and best practice care models.

Job Summary:

The Medical Case Manager will function as an integral member of an interdisciplinary team, which may include medical providers, behavioral health clinicians, medication access specialists, clinical pharmacists, and supportive services staff. Your primary responsibilities will include:

  • Providing medical case management services to identified patients, following medical case management guidelines established by HRSA/Ryan White and the Metropolitan Atlanta HIV Health Services Planning Council.
  • Completing biopsychosocial assessments with new and returning patients, utilizing required screening tools established by the Metropolitan Atlanta HIV Health Services Planning Council and Ryan White/HRSA.
  • Developing comprehensive Individualized Service Plans (ISPs) and providing ongoing monitoring to assess the efficacy of the plan.
  • Coordinating services to meet the goals of the ISP, including care resources, mental health, substance abuse, housing, nutrition, and transportation.
  • Actively participating as part of an interdisciplinary care team to assess patient care needs and coordinate service referrals.
Requirements:

To be successful in this role, you will need:

  • Strong social work, nursing, or case management skills, including the ability to obtain a comprehensive psychosocial assessment and assist adult clients with the development of an individualized service plan.
  • Excellent interpersonal skills and the ability to communicate effectively in a multi-cultural setting, including target populations, volunteers, and agency staff.
  • Proficient computer skills for word processing, data entry, documentation, and navigation of the Internet.
  • Experience in assessing and developing a care plan.
  • Proficient in time management, organizational, and cultural competence skills.
  • Ability to adapt work style to accommodate both solitary and collaborative tasks.
  • Ability to function independently without close supervision.
Preferred Qualifications:

We prefer candidates with:

  • Social service or nursing degree from an accredited university program.
  • Professional experience working in a medical setting.
  • Bilingual Spanish.
  • Comprehensive and up-to-date knowledge of HIV medical and case management fundamentals.
  • Minimum of one year's previous professional experience working with adults living with HIV.
  • Previous professional experience with Electronic Medical Record (EMR) system documentation.
