Job Summary: We are seeking a compassionate and energetic Licensed Clinical Social Worker (LCSW) to join our team at Long Island Select Healthcare Inc. (LISH). As a key member of our clinical staff, you will provide individual, marital, family, and group psychotherapy to patients referred to our services.

Key Responsibilities:

  • Conduct psychosocial assessments and mental health intake assessments as referred.
  • Maintain close liaison with other care providers, including psychiatrists involved in patient treatment.
  • Prepare initial assessments, treatment care plans, treatment care plan updates, daily notes, and discharge notes in a timely manner.
  • Provide relevant information about the PCMH program to patients, including brochures that describe the values and goals of the program.
  • Attend necessary in-service education programs and maintain licensing status in NYS by completing 36 hours of continuing education (CE) from a NYS Department-approved provider during each three-year registration period.

Requirements:

  • Master's Degree from an accredited School of Social Work in the United States.
  • Minimum 2 years' clinical experience.
  • NYS Licensed Clinical Social Worker.
  • Maintain licensing status in NYS by completing 36 hours of continuing education (CE) from a NYS Department-approved provider during each three-year registration period.
