Advanced Practice Nurse

We are currently seeking a dedicated and skilled Advanced Practice Nurse to join our esteemed team at Select Specialty Hospital, a division of Select Medical. Our facilities specialize in the care of patients recovering from critical illnesses and those requiring extended hospital stays.

Key Responsibilities

The Advanced Practice Nurse plays a vital role as part of the primary care team, primarily responsible for managing patients with long-term acute illnesses. This role involves close collaboration with attending physicians to ensure comprehensive patient care.

Specific duties include:
Delivering direct patient care based on evidence-based practice guidelines. Engaging in daily patient rounds with physician teams. Presenting a comprehensive patient report every 24 hours, collaborating on care plans, and monitoring patient progress throughout the day. Following up on treatment interventions, consultations, and assessing risks associated with complex patient care. Managing discharges and new admissions, providing updates to physicians as necessary. Acting as the first responder in cases of patient status changes requiring immediate action. Developing and implementing treatment plans for acute illness management in partnership with attending physicians. Conducting health assessments, performing physical examinations, gathering and interpreting clinical data, and initiating appropriate interventions. Communicating patient outcomes to attending physicians as required. Offering education and leadership to nursing staff when appropriate. Participating in performance improvement initiatives.

Qualifications

Minimum requirements:
Current state licensure as an Advanced Practice Nurse is required. Valid Basic Life Support (BLS) certification required upon hire. Advanced Cardiac Life Support (ACLS) certification required within six months of hire.Preferred qualifications that will enhance your success: Family Nurse Practitioner (FNP) or Adult-Geriatric Nurse Practitioner with acute care experience or Acute Care Nurse Practitioner (ACNP) preferred. Previous experience in a hospital environment is preferred.
