Job Summary:
Geisinger Health System is seeking a skilled Patient Navigator to join our team. As a Patient Navigator, you will play a vital role in coordinating patient care and maximizing patient satisfaction and experience.

Key Responsibilities:
  • Partner with care providers to serve as a liaison and ensure effective communication with patients.
  • Provide discharge information and ensure a smooth transition at discharge to home or other facilities as required.
  • Ensure additional resources are made available for the patient as needed.
  • Effectively collaborate with other healthcare providers and non-Geisinger employees in coordinating and implementing the patient's plan of care.
  • Maintain systems and processes to assure timely follow-up and surveillance of a defined group of patients.
  • Serve as a liaison with clinical and administrative personnel.
  • Establish a point of contact for referring providers to create immediate access to provider templates.
  • Develop and implement efficient patient navigational procedures.
  • Investigate patient concerns, including billing, appointment access, perception of care, physician issues, and communication problems.
  • Develop, maintain, and audit standard operating procedures of patient access and utilization process.
  • Provide administrative support as needed.

Requirements:
  • High School Diploma or Equivalent (GED)
  • Minimum of 2 years of related work experience
