Key Responsibilities:
  • Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams.
  • Provide comprehensive nursing care, including admission, case management, and follow-up visits, to ensure optimal patient outcomes.
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and address patient needs.
  • Conduct thorough assessments, identify areas for improvement, and revise care plans as necessary to ensure patient safety and satisfaction.
  • Supervise and educate Home Health Aides, Certified Home Health Aides, and Personal Care Workers to ensure high-quality care delivery.
  • Document patient progress, communicate effectively with healthcare teams, and maintain accurate records to ensure seamless care coordination.
Requirements:
  • Diploma, Associate, or Bachelor's Degree in Nursing
  • Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational, communication, and interpersonal skills
  • Valid driver's license, auto insurance, and reliable transportation
