As a skilled Home Health Registered Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive admission, case management, and follow-up skilled nursing visits to home health patients.
  • Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams.
  • Confer with physicians to develop and revise treatment plans, ensuring seamless care coordination.
  • Coordinate with multidisciplinary teams, including physical therapists, occupational therapists, and home health aides, to deliver holistic care.
  • Monitor patient progress, report to physicians and Clinical Managers, and make adjustments to care plans as needed.
  • Supervise and oversee the work of home health aides and personal care workers, ensuring high-quality care and documentation.
  • Prepare patients for discharge, ensuring a smooth transition to the next level of care.
  • Maintain accurate and comprehensive medical documentation, including case conferences, patient contacts, and care plan updates.
Requirements:

• Diploma, Associate, or Bachelor's Degree in Nursing

• Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute settings

• Strong organizational and communication skills, with the ability to work independently and as part of a team

• Current and unrestricted Registered Nurse licensure, CPR certification, and valid driver's license

• Home health experience a plus, but not required
