Job Summary

We are seeking a highly skilled and compassionate Hospice Community Liaison to join our team at Yolo Hospice Inc. As a key member of our organization, you will establish and build positive relationships with current and potential referral partners, funders, and community organizations.

Key Responsibilities
  1. Relationship Building: Establish and strengthen relationships with community partners, identifying opportunities for collaboration and partnership on programming or education.
  2. Community Outreach: Develop and implement strategies to increase visibility and awareness of our services among healthcare professionals, community organizations, and seniors.
  3. Program Development: Collaborate with the Director of Business Development to develop and implement new niche services and programming that will help grow census and meet organizational goals.
  4. Customer Service: Foster a culture of platinum-standard customer service within and outside the organization, ensuring exceptional experiences for patients, families, and referral partners.
  5. Education and Training: Serve as a principle community educator, providing education and training on end-of-life issues and hospice care to healthcare professionals, community organizations, and seniors.
  6. Philanthropy and Fundraising: Identify and facilitate opportunities for philanthropic support, working closely with the Development team to secure funding and resources.
Requirements
  1. Education: A four-year college degree in healthcare, Gerontology, or a related field is preferred.
  2. Experience: At least one year of experience working with seniors or senior organizations, with a proven track record of working successfully as a relationship manager.
  3. Skills: Excellent communication and interpersonal skills, with the ability to work effectively with community physicians, healthcare providers, and other stakeholders.
  4. Personal Qualities: Self-motivated, autonomous, and able to work independently, with a high level of comfort functioning as a professional sales person.

Yearly salary: $80,000 - $100,000. Potential bonus: up to $60,000.
