We are seeking a skilled Social Worker to join our team at Yolo Hospice Inc. As a Social Worker, you will play a vital role in providing psychosocial support and counseling to patients, families, and caregivers.
Key Responsibilities- Conduct thorough bio-psychosocial assessments to identify patient and family needs.
- Develop and implement individualized plans of care to address psychosocial issues and coordinate community resources.
- Provide direct clinical services, including counseling and therapy, to patients and families.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Identify and coordinate community referrals to support patient and family needs.
- Participate in weekly IDT meetings to discuss patient progress and care plans.
- Provide bereavement and anticipatory grief counseling to patients and families.
- Bachelor of Social Work degree.
- Knowledge of group process and ability to facilitate interdisciplinary approaches.
- Excellent written and verbal communication skills.
- Knowledge of community resources and ability to coordinate referrals.
- Knowledge of child and elder abuse reporting requirements and relevant legislation.
- Electronic medical records experience preferred.
