Position Summary: Under the guidance of the Nurse Case Manager and the oversight of the Aide Supervisor, you will deliver essential patient care services as outlined in your responsibilities. This role involves assisting adult clients with mobility and personal care needs. You will also maintain accurate records of health care activities in compliance with regulatory standards.

Key Responsibilities

Performance Standards:

  1. Deliver direct patient care services.
  2. Assist in lifting, transferring, and transporting adult clients.
  3. Provide personal care and maintain records of health care activities as mandated by applicable regulations.
  4. Flexibility in working hours is required.

Qualifications

Education: High School Diploma or GED

Experience: A minimum of one year of experience in a healthcare environment is preferred.

H.H.A: Must have worked as a nurse's aide within the last twenty-four months; or

Must pass the competency test as per agency guidelines.

C.N.A.: Nurse Aide Registration as mandated by state regulations.
