Position Overview
We are looking for a dedicated and empathetic Licensed Practical Nurse (LPN) to become a vital part of our healthcare team. In this position, you will play a key role in coordinating patient care services, administering vaccinations, and facilitating our remote patient monitoring initiatives.

Core Responsibilities:
  • Organize patient appointments for pharmacist consultations and vaccination services.
  • Administer a variety of vaccines under the guidance of pharmacists.
  • Support the execution and oversight of our remote patient monitoring program.
  • Conduct basic patient evaluations and document vital signs.
  • Provide education to patients regarding medication usage, potential side effects, and health maintenance strategies.
  • Keep accurate and current patient records.
  • Work collaboratively with pharmacists and other healthcare professionals to ensure high-quality patient care.
  • Assist in managing inventory for medical supplies and vaccines.
  • Adhere to all applicable healthcare regulations and safety standards.

Qualifications:
  • Active LPN license in good standing.
  • At least 2 years of experience in a healthcare environment.
  • Familiarity with electronic health record systems and basic computer proficiency.
  • Strong organizational and time management capabilities.
  • Exceptional communication and interpersonal skills.
  • Experience in vaccine administration is preferred.
  • Understanding of remote patient monitoring technologies is advantageous.
