Job Title: Home Health Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Responsibilities:
  • Assess and evaluate patient needs, developing personalized care plans to meet their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with healthcare providers, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress, updates, and outcomes in a timely and accurate manner.
  • Identify teaching needs and develop strategies to educate patients and their families on disease management, self-care, and health promotion.
Qualifications:
  • RN with a Bachelor's degree in Nursing, preferred.
  • 1+ year of supervised clinical experience as a Registered Nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills and proficiency in electronic medical records.
  • Excellent organizational and interpersonal skills.
