Job Title: Home Health Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Montclair, New Jersey.

About the Role:

As a Home Health Registered Nurse, you will be responsible for providing high-quality nursing care to patients in their homes, promoting independence, and improving their overall well-being.

Responsibilities:
  • Assess and evaluate patients' needs, developing personalized care plans to meet their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Monitor patients' progress, making adjustments to care plans as needed.
  • Document all patient interactions, care plans, and outcomes in a timely and accurate manner.
Requirements:
  • Active New Jersey RN License
  • Bachelor's degree in Nursing preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
  • Car and valid driver's license
