We are seeking a Comprehensive Care Provider to join our team in Los Angeles, Riverside, San Bernardino, Northern CA, Monterey, Merced.

This role will provide holistic and quality care to members enrolled in managed care plans, including closing any identified open care gaps, Health Risk Social Needs gaps, and re-engaging members with primary care services and support to improve member quality of life and decrease total cost of care.

Key Responsibilities:

  • Work effectively with individuals and families to assist them in solving physical, mental, and social health problems.
  • Collaborate with physician practice leaders to ensure high-quality patient care.
  • Lead a health team to deliver comprehensive care services.
  • Communicate effectively with diverse populations, including physicians, employees, patients, and families.
  • Initiate health care treatment for health care management, therapeutic interventions, and prescribing medication when necessary.

Qualifications:

  • Licensed in the state of California as a Nurse Practitioner or licensed in the state of California as a Physician Assistant.
  • Holds National Board Certification.
  • Graduate from an accredited Master's or Doctoral Program.
  • New graduate applicants welcome; preferred at least 1-3 years prior hands-on work experience in healthcare management and patient care.

Salary Range: $135,000 - $200,000 per year
