Key Responsibilities
  • Assessment and Evaluation: Conduct thorough assessments of residents' status, create and implement individualized care plans, and regularly monitor and re-evaluate residents' needs.
  • Medication Management: Transcribe, initiate, and sustain the implementation of orders for medications and treatments as prescribed by physicians.
  • Observation and Reporting: Observe residents for changes in condition, report these changes to physicians and supervisory personnel, and maintain accurate and relevant clinical notes and documentation.
  • Medication Administration: Administer prescribed medications and treatments in accordance with approved nursing techniques.
  • Documentation and Communication: Complete, maintain, and timely submit accurate and relevant clinical notes and documentation regarding residents' condition and care given.
  • Admission and Discharge: Assist with admission, re-admission, and discharge processes as needed.
  • Supervision and Delegation: Supervise and delegate activities of Certified Nursing Assistants (CNAs), Certified Medical Assistants (CMAs), Medication Assistants (MAs), and Licensed Practical Nurses (LPNs).
  • Orienting New Staff: Assist in orienting new nursing staff by observing their practices, interpreting Congregational Home policies and procedures, training, and coaching.
  • Mealtime Supervision: Supervise mealtime and feeding of residents to maintain adequate nutrition and fluid intake.
Requirements
  • Education and Licensure: Current RN license in the state of WI.
  • Experience: Previous experience in a geriatric, long-term care facility preferred.
  • Skills and Attributes: Ability to work independently or with a team, conduct oneself in a caring, sensitive, and safe manner, and promote positive relations and rapport with residents and visitors.
