LTSS Service Coordinator Clinician

We are seeking a highly skilled LTSS Service Coordinator Clinician to join our team at Elevance Health. As a key member of our care coordination team, you will work under the direction of a registered nurse to provide comprehensive care coordination services to our members.
Key Responsibilities- Assist the responsible RN with telephonic or face-to-face assessments to identify, evaluate, coordinate, and manage member needs, including physical health, behavioral health, social services, and long-term services and supports.
- Assist the responsible RN in identifying members at high risk for complications and obtain clinical data as directed.
- Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities, working in conjunction with the RN, member, and healthcare team to ensure cost-effective and efficient utilization of health benefits.
- Utilize decision-making skills to determine the current needs of the member, requiring an understanding of disease processes and terminology, as well as the application of clinical guidelines.
- Requires an LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law and a nursing diploma or AS in nursing or a related field.
- Minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service coordinator or similar role.
- Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
- Proficiency in Microsoft Office (Excel, Word, Outlook, etc.) strongly preferred.
- Case management experience strongly preferred.
- Home health experience strongly preferred.
- Medicaid experience strongly preferred.
- Discharge planning experience strongly preferred.
- MA/MS in Health/Nursing preferred.
