Job Summary

Rosecrance Inc. is seeking a highly skilled and compassionate Community Support Team Lead to join our team of dedicated professionals. As a key member of our clinical team, you will provide case management services, crisis intervention, and skills training to individuals with serious mental illness.

Key Responsibilities:

  • Manage a caseload of clients with complex mental health needs
  • Provide crisis intervention and support to individuals in crisis
  • Develop and implement individualized treatment plans
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Document client progress and outcomes

Requirements:

  • Master's degree in social work, sociology, psychology, or related field
  • Experience in mental health case management and crisis intervention
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment
  • Valid driver's license
