Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our Weekend Program team. As a Registered Nurse at The Oaks at Jamestown, you will play a vital role in delivering high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure their physical, emotional, and social needs are met.
Key Responsibilities- Provide Direct Care: Develop and implement individualized care plans, administer medications, and provide wound care and other treatments as needed.
- Lead Interdisciplinary Team: Collaborate with our team of healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care and support for our residents.
- Coordinate Care Transitions: Facilitate smooth transitions between care settings, ensuring continuity of care and minimizing disruptions to our residents' lives.
- Monitor and Report: Continuously monitor our residents' conditions, reporting any changes or concerns to our medical director and other relevant team members.
- Education and Licensure: Hold a current Registered Nurse (RN) license in the state of Michigan and a degree in nursing from an accredited institution.
- Experience: Possess at least one year of experience in long-term care or a related field, with a strong understanding of gerontology and the needs of older adults.
- Skills and Abilities: Demonstrate excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
