Job Summary

We are seeking a highly skilled Family Nurse Practitioner to join our team at Southeast Community Health Systems. As a Family Nurse Practitioner, you will be responsible for providing primary healthcare to patients and families, focusing on health maintenance, disease prevention, patient education, and counseling.

Key Responsibilities:

  • Perform physical exams on patients of all ages, from neonates to the geriatric population.
  • Obtain and document patient medical history, identifying health risk factors.
  • Diagnose and treat common acute illnesses, as well as chronic conditions such as diabetes and hypertension.
  • Order and interpret diagnostic tests within established scope of practice, consulting with medical director or specialists as needed.
  • Provide well-childcare, women's healthcare, family planning, and prenatal care, as approved under delineation of privileges.
  • Prescribe medications and non-pharmacological therapies based on patient diagnoses, health history, and age.
  • Encourage positive health behaviors through education and counseling.

Requirements:

  • Master's degree from an accredited Nurse Practitioner's Program.
  • Current Advanced Practice Registered Nurse license in the State of Louisiana (or valid compact state) in good standing.
  • Proficiency with Microsoft Office applications (Word, Excel, Outlook, PowerPoint).
  • Ability to communicate in English, both verbally and in writing.
  • BLS/ACLS Certification required.
  • Current, valid DEA and CDS licenses required (for applicable specialties).
