We are seeking an experienced MDS Coordinator to join our team at Southview Acres Health Care Center.

Estimated Salary: $65,000 - $85,000 per year
Key Responsibilities- Conduct thorough patient assessments to identify individual needs and develop tailored care plans.
- Ensure compliance with Medicare and Medicaid regulations by accurately completing required documentation and implementing process improvements.
- Collaborate with the interdisciplinary team to provide expert guidance and support, fostering a culture of collaboration and excellence.
- Develop and maintain relationships with healthcare professionals, vendors, and community partners to enhance the quality of care and services provided.
- Participate in ongoing education and training to stay current on industry developments, best practices, and regulatory changes.
- Possess a degree in nursing or a related field (Associate's or Bachelor's degree preferred).
- Hold current certification as a Medical Social Worker (MSW) or a Nurse Practitioner (NP).
- Demonstrate excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, and healthcare professionals.
- Exhibit strong analytical and problem-solving skills, with the capacity to think critically and develop creative solutions.
