Job Summary:

Holy Name Medical Center is seeking a compassionate and skilled Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families in need of end-of-life care.

Key Responsibilities:

  • Assessment and Planning: Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and develop individualized care plans to address these needs.
  • Implementation and Evaluation: Implement care plans and evaluate their effectiveness, making adjustments as needed to ensure optimal patient outcomes.
  • Patient Education: Provide education and support to patients and their families on end-of-life care, pain management, and other relevant topics.
  • Collaboration and Communication: Work collaboratively with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure comprehensive patient care.
  • Leadership and Mentoring: Assume charge nurse responsibilities and mentor new staff members to ensure a smooth transition and high-quality patient care.

Requirements:

  • Education: Graduate of an accredited school of nursing with a current New Jersey license. B.S.N. degree preferred.
  • Experience: Prefer a minimum one-year current clinical experience required, hospice experience preferred.
  • Certification: Current NJ License. Required BLS through American Heart Association. National certification related to specialty area preferred.
