About the Role

We are seeking a Licensed Vocational Nurse (LVN) to join our team as a Home Health LVN - PRN. As an LVN, you will visit patients in their homes and provide nursing care under the direction of RNs and in compliance with physicians' orders.

Responsibilities
  • Provide nursing care to patients in their homes, under the direction of RNs and in compliance with physicians' orders.
  • Develop and implement individualized care plans for patients.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Collaborate with RNs and other healthcare professionals to ensure high-quality patient care.
Qualifications
  • Current licensure as a Licensed Vocational Nurse in the state of employment.
  • Valid state driver's license and automobile liability insurance.
  • Dependable transportation and ability to drive in all types of weather conditions.
