Job Title: LVN Charge Nurse

Regency Integrated Health Services is seeking a skilled and compassionate LVN Charge Nurse to join our team at Bastrop Lost Pines Nursing and Rehabilitation Center in Bastrop, TX.

Job Summary:

The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This includes observing, assessing, and documenting resident conditions, implementing care plans, administering medications, and conducting rounds with physicians.

Responsibilities:
  • Observe and assess resident conditions, documenting and reporting changes
  • Implement care plans as directed by physicians and nursing staff
  • Administer medications as prescribed by physicians
  • Conduct rounds with physicians and record visits in clinical records
  • Perform narcotic reconciliation with incoming and outgoing shifts
  • Identify significant changes in resident conditions and take necessary action
  • Oversee and monitor aides
  • Initiate and complete Incident and Accident Reports as per facility policy
  • Discharge residents as per facility policy
  • Complete Nursing Assessments for Physician Calls prior to contacting physicians
  • Communicate with resident's point of contact regarding status updates or changes in condition
Qualifications:
  • Active Licensed Vocational Nurse in the state of Texas in good standing
  • Completed an LVN program at an accredited vocational school or college
  • Experience working in a skilled nursing facility preferred
