Job Summary
The Facility Maine Behavioral Healthcare Assertive Community Treatment (ACT) team is dedicated to providing high-quality mental health rehabilitation services, supporting adults and families to reduce hospitalization and crisis episodes. The ACT Clinical Case Manager, operating under the guidance of the ACT Team Program Manager, delivers essential services, primarily in the clients' living situations, including crisis intervention, case management, individual coaching, monitoring for medication adherence, and skills training. In the community and on-call situations, the case manager exercises the experience and judgment required to make sound clinical decisions independently.
Key Responsibilities
- Deliver mental health rehabilitation services to clients in their living situations, including crisis intervention, case management, and individual coaching.
- Monitor clients' medication adherence and provide skills training to support their recovery.
- Exercise sound clinical judgment in community and on-call situations to make informed decisions.
- Collaborate with the ACT Team Program Manager to ensure client service delivery aligns with regulatory, accreditation, and best practice standards.
Requirements
- Bachelor's degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, corrections, rehabilitation, nursing, or a closely related field.
- 2+ years of experience in the same areas of specialization; or 5 years of experience working with people diagnosed with severe mental illness.
- MHRT-C certification required unless clinically licensed; Eligibility for LSW licensure preferred; Valid Driver's license and appropriate level of automobile liability insurance.
- 5+ years of experience working with a broad spectrum of mentally ill clients is preferred.
- Excellent computer skills, including Microsoft Word, Excel, and electronic consumer record software.
- Knowledge and commitment to abide by the Code of Ethics of the National Association of Social Workers.
