Purpose of Position:

Serves as the primary clinician for individuals with substance use disorders, promoting recovery and self-sufficiency through culturally relevant and medically monitored treatment programs.

Develops and implements clinically sound treatment plans, case management, and care coordination services for patients in Substance Use Disorder (SUD) treatment programs.

Responsibilities:
  • Conducts initial screenings and assessments of patients, performs ongoing toxicology screenings, and develops psycho-social histories.
  • Formulates diagnostic impressions, prepares screening, intake, and assessment reports, and facilitates individual, group, and family counseling.
  • Makes decisions regarding appropriate Level of Care and maintains required records, using patient criteria tools and clinical judgment.
  • Develops and monitors treatment/recovery plans, coordinates case and care management services, and initiates discharge and transition planning.
Requirements:
  • Valid CASAC - Level 1 Certificate issued by OASAS.
  • One year of full-time, progressively responsible experience in SUD assessment, treatment planning, individual and group counseling, case and care management, and other functions related to SUD prevention and treatment.
