Job Summary

We are seeking a highly skilled and compassionate Case Manager Professional to join our team at St. Clair County Community Mental Health. As a key member of our team, you will play a vital role in providing direct case management services to individuals served, coordinating and linking them with necessary resources and services.

Key Responsibilities
  • Conduct and document assessments of individuals served to identify their needs and develop personalized plans for service.
  • Maintain accurate and timely records of individuals served, including progress notes and service plans.
  • Develop and implement effective case management plans, ensuring seamless coordination with other service providers and stakeholders.
  • Provide observation and monitoring of service implementation, identifying areas for improvement and making recommendations as needed.
  • Develop and maintain community connections, linking individuals served with schools, physicians, courts, and other community programs and agencies.
Requirements
  • Bachelor's Degree in Social Work, Education, Special Education, Health Education, Health Administration, Psychology, Human Services, Criminal Justice, or related field.
  • Valid driver's license.
  • At least one year of experience in a job-related field or evidence of such placements during schooling.
  • Demonstrated ability to use Microsoft Office applications and Windows 10 programs.
  • Must have access to transportation.
  • Must qualify as QMHP, QIDP, or CMHP, according to program assignment.
Preferred Qualifications
  • Licensed Bachelor Social Worker.
  • At least three years of experience in a job-related field.
  • Demonstration of proficiency and experience working on an electronic health record system.
  • Knowledge of CMH system and operations.
  • Lived experience with behavioral health issues.
Work Environment

This position requires the ability to work in a fast-paced environment, with frequent standing and walking. The employee must occasionally lift and/or move up to 25 pounds. The work environment is comfortable, with occasional noise and limited exposure to disagreeable work elements.
