We are seeking a skilled and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care and support to patients in their homes.
Key Responsibilities- Provide direct patient care under the direction of a Registered Nurse and according to the Aide Plan of Care (POC)
- Assist patients with self-administered medications, including opening bottle caps, reading labels, and observing medication intake
- Take accurate temperature, pulse, and blood pressure measurements and report abnormal results to the supervisor
- Help patients maintain good personal hygiene through bathing, grooming, skin care, and other activities
- Assist with feeding patients, including communicating basic nutrition principles and observing food and fluid intake
- Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation
- Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care
- High school diploma or equivalent
- Completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
- Must meet applicable state certification requirements
- Valid driver's license, auto insurance, and reliable transportation
- At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency
