About the Role

We are seeking an experienced Outpatient Physical Therapist - Patient Care Specialist for full-time or part-time opportunities performing outpatient visits in assisted living communities.

Job Overview

This is a dynamic role that requires strong clinical skills and excellent patient communication. As an Outpatient Physical Therapist, you will work closely with our multidisciplinary team to provide comprehensive care to adult and geriatric clients on a per-visit basis.

Key Responsibilities
  • Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans.
  • Perform diagnostic tests and measurements to evaluate patients' mobility, strength, and balance.
  • Implement manual therapeutic exercises, gait training, and other interventions to promote patients' independence and mobility.
  • Educate patients and their families on proper techniques for self-care and injury prevention.
Requirements
  • A current VA Physical Therapist license is required.
  • Prior experience as an Outpatient Physical Therapist is preferred but not necessary.
  • Excellent communication and interpersonal skills are essential for success in this role.
